The Manager of Population Health Operations leads a team of care coordination staff to ensure optimal care for patients, including those with complex medical conditions, patients being discharged from the hospital and patients needing preventive screenings.  The manager maintains clinical standards of care based on care management workflows aligned with an integrated, cross disciplinary team-based approach.  The manager collaborates with administrators, medical staff, and other disciplines to ensure alignment with evidence-based population health management practices (NCQA PCMH and Health Equity Accreditations).

  • Manages and supervises RN care coordinators and care gap coordinators effectively and efficiently including hiring, performance management, staff onboarding and development, and disciplinary actions.
  • Manages key population health management programs such as Complex Care Management (CCM), Transitional Care Management (TCM), and Quality Improvement for Value-based Care Contracts including, but not limited to workflow optimization, program reporting, and outcomes measurement.
  • Supports implementation of quality improvement models, including management of performance standards consistent with ambulatory quality initiatives.
  • Understands and utilizes healthcare knowledge and experience to perform various independent analyses to support the design, implementation, and evaluation of population health management programs.
  • Uses critical thinking skills and innovation to improve processes and care for patients served.

·       Participates in certification, accreditation, and audit procedures to support optimal performance of the team.

·       Collaborates and maintains effective relationships and liaises with leaders across Grady Health System, community partners, managed care organizations, and medical staff in pursuit of continuously improving patient outcomes.

·       Effectively represent the department in meetings, conferences and other activities related to population health care management operations and related domains as assigned.

·       Maintains an organized approach to work and the ability to function at a high degree of independence, urgency, accuracy, thoroughness, and attention to detail.

Qualifications:

·       Active Nursing license in the state of GA required.

·       Bachelor’s degree in nursing preferred with 5 years of clinical experience in ambulatory care management, progressive leadership preferred.

·       Experience supporting NCQA accreditation and Certification in case management (CCM).

·       Experience and knowledge of state and federal healthcare regulations, including reimbursement and coverage policies.

·       Excellent written and verbal communication skills, including ability to present to large or small groups.

·       Ability to work in a team environment and manage multiple deadlines and prioritize assignments.

·       Well organized with a high degree of accuracy and attention to detail.
