SUMMARY
Grady Health System is currently seeking a compassionate and experienced Oncology Nurse Navigator to join our multidisciplinary cancer care team. This role is pivotal in guiding newly diagnosed cancer patients through the complexities of cancer care by serving as a consistent point of contact and resource throughout the continuum of care—from diagnosis through treatment, survivorship, or transition to end-of-life care.
The ideal candidate will have experience in oncology or genitourinary (GU) clinics, and hospital case management, and demonstrate a deep commitment to patient-centered care.

KEY RESPONSIBILITIES
  • Serve as a dedicated navigator for newly diagnosed oncology patients, ensuring timely initiation and coordination of care.
  • Identify and help resolve barriers to treatment, including logistical, financial, emotional, and educational challenges.
  • Educate patients, caregivers, and support systems on diagnosis, treatment options (chemotherapy, radiation, surgery), clinical trials, and ongoing care expectations.
  • Maintain up-to-date knowledge of evolving cancer therapies and care practices, with a commitment to continuous learning.
  • Initiate contact with new patients before their first oncology clinic appointment to provide education, reassurance, and support.
  • Attend multidisciplinary care meetings and communicate with providers to ensure coordinated, comprehensive care planning.
  • Deliver site-specific navigation based on cancer type, serving as an expert and advocate throughout all phases of treatment.
  • Provide emotional support and culturally competent care throughout the care journey—from diagnosis to survivorship or hospice.
  • Cross-cover other nurse navigators as needed, supporting a collaborative and cohesive team environment.
  • Maintain accurate and timely documentation of patient encounters and interventions in accordance with system standards.

QUALIFICATIONS
Required:
  • BSN Degree
  • Current licensure as a Registered Nurse in the State of Georgia
  • Three years of direct nursing experience and demonstrated clinical competence
  • Current AHA BLS certification
  • Demonstrate competence in clinical specialty (Oncology)
  • Utilize advanced clinical thinking and analytical skills that contribute to patient adherence of treatment protocols and symptom management
  • Competent in use of Electronic Medical Records
Preferred:
  • Oncology Nurse Certification (OCN)
  • Hospital-based case management experience.
  • Oncology or GU Clinic experience
