SUMMARY

The Patient Navigator serves as a liaison between patients, healthcare providers, and external service agencies to reduce barriers to care, address social determinants of health, and assure the patient receives the care they need when they need it. Works with the patient to identify barriers to care and develop a comprehensive and goal-oriented plan. Facilitates patient appointments as needed, including those made with healthcare providers, labs, diagnostic areas, and Physical Therapy services.  Provides assistance with clinical and supportive care services offered within the Grady Health system.

  • Schedules clinic pre-operative and post-operative visits.
  • Enters timely documentation of telephonic or face to face interaction with patients.
  • Collaborate with appropriate clinical care teams to ensure coordination of services for orthopedic care are scheduled and followed through to ensure the patient receives necessary and timely services throughout the care continuum.
  • Assists patient with obtaining completed paperwork associated with their clinical visit.
  • Communicates daily with Patient Financial Assistance Counselor, Social Work, and Pre-operative scheduling coordinator to assure pre-surgical clearance process is completed for patients scheduled for surgery.
  • Provides updates to patient educational material associated with surgical pre-operative and post-operative processes. Documents patient specific comments in appointment notes for efficient and effective scheduling.
  • Coordinates surgical scheduling through the direction of Attending Surgeon(s). Orients patients to the health system, orthopedic clinic, and ancillary departments.
  • Assists with progression of ambulatory referrals for care coordination and schedules accordingly in Epic.
  • Assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate community services, and medications.
  • Communicates patients concerns to the clinical care team. Enters orders where necessary.
  • Participates in clinic meetings and initiatives as needed.
  • Recognizes through communication queues when to direct patients to seek urgent or emergent care.

Required:
Associate's Degree or two years college level coursework

Three (3) years job related experience in a healthcare environment

Preferred:
Bachelor's Degree
