JOB SUMMARY:

The Care Coordination Advanced Educator develop and provides education, training, updates and follow up to team on the New Care Coordination Model. Additionally, identify areas of where there is learning opportunity and provide to leadership recommendations.

SKILLS/CERT/LICENSES:

Current Georgia License as a Registered Professional Nurse

Basic Life Support (BLS) certification

Must possess basic computer skills and be proficient in use of Microsoft Office

Preferred: Knowledge of audiovisual equipment set-up

EDUCATION:

Bachelor’s Degree in healthcare related field.

EXPERIENCE:

Three (3) or more years of clinical experience in education and training.
